Deep Vein Thrombosis Treatment

Deep vein thrombosis treatment in Oakland, NJ and Fairfield, NJ

Deep vein thrombosis (DVT) is a blood clot (thrombus) that forms in a deep vein, most commonly in the legs (thigh or calf). Treatment for DVT focuses on preventing the clot from growing, stopping it from breaking off and causing a pulmonary embolism (PE), and reducing the risk of future clots or long-term complications.

Diagnosing Deep Vein Thrombosis

To diagnose DVT, our board-certified vein specialists will conduct a thorough physical exam checking each leg for symptoms of:

  • Leg swelling
  • Tenderness or unexplained leg pain
  • Changes in skin color
  • Hardening of vein under the skin

Main Tests for DTV Diagnosis

dvt ultrasound testing in Oakland, NJ and Fairfield, NJ

To diagnose deep vein thrombosis (DVT), doctors use a combination of physical exam findings, risk assessment, and imaging/lab tests.

Duplex Ultrasound (First-line test)

  • Uses sound waves to visualize blood flow in the veins.
  • Can detect clot location and whether blood flow is blocked.
  • Most common and reliable test for DVT in the legs.

D-dimer Blood Test

  • Measures fragments from clot breakdown in the blood.
  • A negative D-dimer can help rule out DVT in low-risk patients.
  • A positive result doesn’t confirm DVT (can be elevated in infection, cancer, surgery, pregnancy, etc.), so imaging is needed.

Venography (Contrast X-ray)

  • Dye is injected into a vein, then X-rays are taken.
  • Rarely used today due to discomfort and invasiveness, but very accurate.

CT or MR Venography

  • CT venography: special CT scan with contrast to view veins.
  • MR venography: MRI with contrast, often used if ultrasound is inconclusive or clot is in the pelvis/abdomen.

Other Tests

  • Wells Score (Clinical Prediction Tool): Assesses likelihood of DVT before testing.
  • Blood tests for thrombophilia: May be done in patients with recurrent/unexplained DVT.

Main Treatments for DVT

If you are diagnosed with deep vein thrombosis (DVT), treatment is focused on stopping the clot from getting bigger, preventing it from traveling to the lungs, and lowering your risk of new clots in the future. Most patients are treated successfully with medications, but in some cases, other procedures or supportive therapies may be needed.

Anticoagulants (Blood Thinners) – First-line Treatment

These don’t dissolve the clot but prevent it from growing and new clots from forming.

  • Heparin (IV or injection) – acts quickly, often used in hospitals at diagnosis.
  • Low Molecular Weight Heparin (LMWH) – e.g., enoxaparin; often given by injection.
  • Warfarin – requires blood monitoring (INR).
  • DOACs (Direct Oral Anticoagulants) – e.g., apixaban (Eliquis), rivaroxaban (Xarelto), dabigatran; often preferred for ease of use.

Duration: Usually 3–6 months, longer if risk persists or clot was unprovoked.

Thrombolytic Therapy (Clot Dissolving)

  • Used in severe cases (extensive clot, limb-threatening, or massive PE).
  • Medications like tPA are given through IV or catheter directly into the clot.
  • Higher risk of bleeding, so reserved for emergencies.

Inferior Vena Cava (IVC) Filter

  • A small device placed in the large vein (vena cava) to catch clots before they reach the lungs.
  • Considered if anticoagulants can’t be used or are ineffective.

Compression Therapy

  • Graduated compression stockings reduce swelling and pain.
  • Help prevent post-thrombotic syndrome (chronic pain, swelling, ulcers).

Lifestyle & Supportive Measures

  • Elevate the leg when resting.
  • Stay mobile—avoid prolonged sitting or standing.
  • Maintain hydration.
  • Address risk factors: weight management, quitting smoking, treating underlying conditions.

Most patients are treated successfully with anticoagulants for several months. In severe cases, doctors may use thrombolysis, thrombectomy, or IVC filters. Compression stockings and lifestyle changes help reduce long-term complications.

What to Expect After DVT Treatment

Being treated for deep vein thrombosis (DVT) is an important step toward protecting your health. Treatment helps prevent the blood clot from getting larger and lowers the risk of serious complications, like a pulmonary embolism (PE). Here’s what you can expect after treatment:

  • Medications: Continue taking blood thinners as prescribed by your doctor, understanding their purpose is to prevent clots from growing and to reduce the risk of new ones.
  • Stay Active: Get up and move around as much as you safely can, as activity promotes blood flow and helps the body heal.
  • Elevate Your Leg: When resting, raise your leg on a pillow to reduce swelling.
  • Compression Stockings: Wear compression stockings if recommended by your doctor to help manage swelling and improve blood flow in the affected leg.

After DVT treatment, most people recover well with medication, lifestyle changes, and monitoring. Your doctor may order follow-up ultrasounds to check your veins and guide how long you’ll need treatment. Staying consistent with treatment and knowing when to call your doctor are key to preventing future problems.
