Signing up is easy.

STEP 1 Go to our Patient Portal. Get this link from a previous email sent by our practice, from our website, or from a staff  member. Click “Create an Account”.

STEP 2 Select if you are the “Patient” or the “Caregiver or Family Member”, then type in your Date of Birth and Email. Click “Continue”.

STEP 3 Check and open the AthenaHealth email. Click “Create My Account”.

STEP 4 Create your account by entering in the required information. Click “Continue”.
