Virtual Care encompasses two complementary services designed to keep you healthy and connected to your care team: Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). These services allow our medical professionals to support your health between clinic visits. Our goal is to help you stay on track with your care plan and feel your best – everyday.

Remote Patient Monitoring (RPM)
Remote Patient Monitoring allows our clinical team to track your vital signs and health data in real-time using easy-to-use medical devices that you keep at home.
How It Works
We provide you with FDA-approved monitoring devices such as blood pressure cuffs, weight scales, or glucose monitor to take daily or regular readings at home. These devices automatically transmit your readings to our secure system, where our healthcare professionals review your data regularly. We will contact you if your numbers are outside your normal range.
What We Monitor
Blood pressure and heart rate
Weight fluctuations (important for heart failure management)
Oxygen saturation levels
Heart rhythm and ECG readings
Other cardiac-specific measurements as needed
What To Expect
You may receive a monitoring device in the mail or from your provider
We'll show you how to use it
A nurse or care manager may call you each month or when there’s a concern
You’ll still keep your regular doctor’s appointments — this program adds an extra layer of support
Your Experience
Simply use your monitoring devices as instructed—most take just a minute or two each day. Our team receives your data automatically and will contact you if any readings require attention or adjustment to your care plan. You'll have regular check-ins with our nursing staff to discuss your progress and address any questions.

Your blood pressure, weight, pulse, or oxygen is out of range

You’ve missed device readings for several days in a row
Chronic Care Management (CCM)
Chronic Care Management provides you with comprehensive, coordinated care for your cardiac condition through regular communication with our dedicated care team.
How It Works
You'll be assigned a care coordinator who works closely with your cardiologist to develop and manage a personalized care plan. Your care team is available to you via phone, secure messaging, or video calls to provide medical guidance, medication management, and support between your office visits.
What To Expect
Monthly (or more often if needed) check-ins with a care team member
Help managing chronic conditions like diabetes, high blood pressure, heart failure, or high cholesterol
Medication review and support
Health coaching and goal setting
Coordination with your doctors and specialists
When We’ll Reach Out
You'll receive regular phone calls from your care coordinator to discuss your health status, symptoms, medications, and any concerns. Between scheduled contacts, you can reach out to your care team whenever questions arise. Everything is documented in your electronic health record to ensure seamless, coordinated care.
